The NP (Nurse Practitioner) is an advanced practice nurse leader functioning in an expanded nursing role as Most Responsible Provider (MRP) for our post-acute patients. The NP-MRP practices autonomously to their full scope of practice, collaborating as necessary with the Internal Medicine and Family Medicine physician teams, with support from the larger interprofessional team.
The NP-MRP identifies solutions for complex healthcare issues for patients within Belleville General Hospital and/or Trenton Memorial Hospital’s post-acute units (rehab, complex continuing care, assess and restore, and/or alternate level of care). The NP-MRP is responsible for admission, discharge, documentation, assessments, diagnoses, ordering and interpreting diagnostic tests, prescribing pharmaceuticals and performing procedures within the legislated scope of practice.
In addition to direct clinical care, the NP-MRP leads the development and implementation of applicable clinical guidelines and protocols, promotes the use of research and evidence-based practice, provides expert support and consultation, and facilitates system change. The NP-MRP demonstrates sound communication skills, champions person and family centered-care, advocates for vulnerable patients and their caregivers, and provides support and education to the interprofessional team.
