Diabetes Education Program

The Diabetes Education Program at Quinte Health works in collaboration with the South East Diabetes Regional Coordination Center and other provincial and regional organizations and agencies.

The model is a consultative service for professionals and individuals clients/patients/families that are living with diabetes.  The diabetes team helps the individual to self-manage their diabetes.

The interprofessional team consists of certified diabetes nurse educators and certified diabetes dietitians.

The Pediatric team also has a pediatrician and social worker.

Referral to Diabetes program is by physician or self-referral from the client/patient/family by calling the diabetes program.
