The Palliative Care Nurse Consultant is part of the larger QHC interprofessional team.

The Palliative Care model is a consultative service for professionals and individual clients/patients/families that experience issues related to pain, symptom management and psycho-social-emotional needs.

These services are provided to the adult inpatient and outpatient populations. Support is also offered to individuals and their families transitioning from palliative to end-of-life care.
