How to access medical records

There are occasions when patients or substitute decision makers request copies of medical records for their own use.  QHC requires a written consent authorizing QHC to release the records.  The following explains the process of obtaining records from QHC hospitals. 

1. All patients/substitute decision makers must provide identification when requesting copies of their own record. 

2. Substitute decision makers must provide proof of legal authority. 

3. All requests must be made in writing. The consent must be original, signed, dated and witnessed. Click here to obtain a copy of the Authorization for Release of Patient InformationYou may print the Quinte Health Care authorization form, complete it, provide contact information and mail it to Quinte Health Care, or you may come directly to Health Records where you will be provided with the form. You will always be required to provide identification. As a courtesy the most responsible physician is notified of the request.

4. A charge for copies of medical records for personal use based on the current Corporate fee schedule will apply; you can contact Health Records for details by calling- 613-969-7400 ext.2280 (Belleville General), 613-392-2541 ext.5429 (Trenton Memorial), 613-476-2181 ext. 4249 (Prince Edward County Memorial) or 613-332-2825 ext. 6225 (North Hastings).  There is no charge for medical records if the record is for ongoing care and the request is for QHC to provide the records to a care provider.
