Diabetes Education & Management Program

The Diabetes and Management Program at QHC works in collaboration with the South East Diabetes Regional Coordination Centre and other provincial and regional organizations and agencies.

The model is a consultative service for professionals and individual clients/patients/families that are living with diabetes. Promoting self-care, the team develops a special management plan for each individual to help them understand how to relieve diabetes symptoms, prevent or treat complications, and improve the quality of life.

The interprofessional team consists of registered nurses and registered dietitians for all the programs and the Pediatric team also has a pediatrician and social worker.

REFERRAL TO DIABETES & MANAGEMENT IS BY PHYSICIAN AND BY SELF-REFERRAL FROM THE CLIENT/PATIENT/FAMILY.
