Registered Dietitians are part of the larger QHC interprofessional team.

The Clinical Nutrition model is a consultative service for professionals and individual clients/patients/families that experience issues related to nutrition.

Assessment, education, care planning and follow-up services are provided to the pediatric and adult inpatient and outpatient populations.

Outpatient services are provided on a one to one basis by a Registered Dietitian.

Registered Dietitian outpatient services are also provided in collaboration with other ambulatory clinics such as Diabetes & Management, Oncology, Rehabilitation Day Hospital, and Children’s Treatment Centre and Stroke Prevention.

REFERRAL TO THE CLINICAL NUTRITION IS BY ALL PROFESSIONALS AND BY SELF-REFERRAL FROM THE CLIENT/PATIENT/FAMILY.
